GUIDELINES 1) All Gluck Fellows and Music Partnership teachers must pass a Tuburculin test. If you have taken a TB test within the last 12 months and have proper documentation, you may submit this for approval. Otherwise, you must schedule an appointment to take a TB test at your local health provider (this may be the Arthur Ashe Student Health and Wellness Center). Keep in mind that TB tests involve TWO appointments, within the span of about three days. 2) All Gluck Fellows and Music Partnership teachers must pass a Department of Justice Background Check. This is done via fingerprinting at the UCPD. Please call (310) 206-8126 to make an appointment between the hours of 7:00 am and 5:00 pm. The Request for Live Scan Service (below under "Forms") should be completed PRIOR to your fingerprinting. Please fill out the personal information in the second box (follow the check-marks). For "Job Title and Department" in the top box, please write in "Musician-Music Partnership" OR "Gluck Performance Program." HANDBOOK
